The Insane is the title behind Insane in the Membrane, a Feat of Strength added in patch 3.1.0 and still live in retail WoW Midnight. It carries 7 reputation criteria: Honored with the Bloodsail Buccaneers, plus Exalted with Booty Bay, Everlook, Gadgetzan and Ratchet - the four Steamwheedle Cartel towns - and Exalted with Ravenholdt and the Darkmoon Faire. Exalted with Shen'dralar was cut from the criteria in patch 4.0.3a and is no longer part of it.
The real numbers. Every Exalted bar is 42,000 reputation from Neutral: 3,000 across Friendly, 6,000 across Honored, 12,000 across Revered and 21,000 to Exalted. So the Feat wants six complete 42,000-point bars plus the first 9,000 with the Bloodsail. Ravenholdt is the ugliest of the six. Syndicate mobs at Stromgarde Keep and Northfold Manor pay 5 rep a kill and stop dead at 11,999/12,000 Honored, and after that the only repeatable source outside the rogue class chain is Junkboxes Needed - 5 Heavy Junkboxes for 75 rep. Closing the last 21,000 that way is 280 turn-ins and 1,400 Heavy Junkboxes, and junkboxes come only from a rogue's Pick Pocket. They can be traded and mailed, but never listed on the auction house.
Bloodsail against Steamwheedle, and why the order matters. The only way to raise Bloodsail rep is to kill Booty Bay faction NPCs - ordinary citizens pay 5, while Bruisers, the bankers and Baron Revilgaz pay 25 - and each kill drains Booty Bay rep and half as much from Ratchet, Gadgetzan and Everlook. The sequence is therefore Bloodsail to Honored first, then all four goblin towns rebuilt to Exalted on Bloodsail and Venture Co. kills. A 2014 hotfix locked every criterion in permanently once it has ever been reached, so the two standings never have to be held at the same time.
The Darkmoon Faire is a calendar, not a grind. Faire quests pay 250 rep apiece and the Faire only runs one week per month, so Exalted normally lands across 4 to 5 monthly Faires unless Darkmoon card and artifact turn-ins accelerate it.
How long it really takes, and how it books. This is not a 10-hour job. Guides and boosters put the full title at roughly 60 to 100+ hours of active farming, spread over several months because of the Faire window. The rate is flat at every rung - 2 hours $26.99, 4 hours $53.99, 6 hours $80.99, 10 hours $134.99 - and a campaign this long is booked as repeat blocks at the same hourly rate rather than squeezed into one. These old-world reputations are character-bound, since Warband sharing covers modern factions and not these, so every bar is earned on the single character you name. Ravenholdt also gates the rogue-only Fangs of the Father legendary chain, which timesaver.gg sells as a separate product - this SKU is the reputation work and the title, not the daggers.
