Halloween: First Results is the Therapist quest that keeps stalling players in the Tarkov Halloween 2026 event. It asks for 25 infected kills, but only infected under the effect of an experimental gas grenade count, and Therapist hands you just three grenades to do it. Threads about it have been running on r/EscapefromTarkov since patch 1.3.0.0 went live on October 29, 2026, and the complaint is usually the same: plenty of kills, almost no quest progress.
Quick answer: Accept First Results from Therapist and you receive 3 RDE-68 gas grenades. Equip a respirator or gas mask, go to Infected Factory or Infected Lab, get somewhere the infected can't reach you, throw the grenade under your position, wait a few seconds for the gas to spread, then kill whatever is standing in it. You need about 8 to 9 counted kills per grenade. More grenades come from two Therapist barters: one for Model 8230 CS gas grenades, one for a blood sample plus construction materials. Reward: +42,000 EXP, 350,000 ₽ and 5 Grizzly medical kits.

What Does Halloween: First Results Ask You to Do?
First Results has one required objective and one optional one, per the tarkov.help quest page:

The maths is the real problem. Three grenades for 25 kills means 8.3 counted kills per grenade. One player on r/EscapeFromTarkovPvE reported reaching 17/25 with two grenades, so three is doable, but only if almost none of your grenades are wasted.
Why Aren't My First Results Kills Counting?
Because a zombie standing near the gas isn't the same as a zombie affected by it. The objective tracks the grenade's status effect, not your location or the smoke on screen.
That's the part players keep hitting. u/momokinou wrote on r/EscapefromTarkov after trying the Gate 3 hallway on Factory: "I can get zombie in the gas but killing them inside doesn't work. I killed ~10 and got only one for my quest."
The community has two explanations, and BSG hasn't documented the exact rule:
- It has to be in the middle of the cloud. u/chevaliergrim argues that infected "dont get a status effect by passing though the gas nades" and that kills only count near the centre.
- The effect takes time to apply. A commenter in the grievances thread compares it to a normal gas grenade: the target has to stay in it for a moment before it's affected.
Both point at the same fix. Don't shoot the moment an infected touches the cloud. Let it stay in the gas for a few seconds, then kill it. Players report that sprinting zombies that run straight through often don't count.
How Do You Finish First Results With 3 Grenades?
This is the method the tarkov.help walkthrough recommends, and it says the whole quest can be done in one raid:
- Equip a respirator or gas mask before you load in. Without one, the gas hurts you too.
- Pick Infected Factory or Infected Lab. These hold the densest infected groups. Factory is the smaller map, and both its PMC exits, Gate 0 and Gate 3, are always open.
- Get above the infected. Tarkov.help suggests crates, pipes or tables where they can't reach you.
- Let a group build underneath you. Don't throw at the first two zombies.
- Throw the grenade under your position and wait a few seconds for the gas to spread.
- Shoot the infected standing in the cloud. Check your quest progress after the first few kills, so you know whether your timing works before the grenade runs out.
Run high-capacity, full-auto firepower. A jam with a pack on you is usually the end of the raid. You should already own the best option: Halloween: Rumors, the first quest of the event, pays out an AA-12 Gen 2 automatic shotgun, 3 drum magazines and 4 packs of SuperFormance HP slugs. Tarkov.help's advice for the later Final Push also applies here: high-damage flesh rounds, a big magazine and no suppressor.
Expect headshots to take more than one hit. u/lotsofop wrote that "PKM or Glock with flesh ammo it doesn't matter they will both require two taps LOL." That's one player's experience rather than a confirmed stat, but plan your ammo around it.
Where Do You Get More RDE-68 Grenades?
From Therapist. Accepting the quest should add two new barters. Tarkov.help notes that if they don't show up, you should restart the game.

Option 2 is the one most players will take. The wiki's Follow-Up Examination page lists where the dead scientists in hazmat suits lie:
- Infected Factory: outside the medical labs, or next to the pumping station on top of the tanks.
- Infected Lab: inside the tunnel to the Sewage Conduit extract, at the intersection near the Blue keycard room, or in the middle of the working area.
Bring Medical tools and hold the action key the whole time. Tarkov.help says the extraction takes 20 to 30 seconds and fails if you let go. u/lotsofop says the sample isn't a 100% spawn and puts the Flea price at about 500,000 ₽ each, so check live prices before you commit.
Where Does First Results Fit in the Halloween Questline?
It's on Therapist's route only. If you side with Skier at the second choice, you won't see it. Here's the order on the Therapist route, with the quest names as tarkov.help and the wiki list them:

Finishing Final Push awards the Triumph of Science achievement. Skier's route ends in its own achievement, and tarkov.help warns that getting the other one means resetting your account. So think before you complete Tightening The Quarantine, because doing that fails A New Vaccine. If you stashed the vaccine for Skier at step 4, you aren't locked out yet: Unexpected Help also leads to A New Vaccine.

Where Do You Get Infected Tissue Samples for Studying the Enemy?
Not from zombies. You can't loot the infected, which is why so many players searching corpses come up empty. Tarkov.help lists these ways to get them:
- Finish First Results first. It unlocks For the Common Goal, and its companion quest with Skier pays out 2 tissue samples, exactly what Studying the Enemy needs. This is the free route, so ignore Studying the Enemy until then.
- Buy them on the Flea Market. That's the only other option on Therapist's route.
- Skier's route earns them through its own quests. The wiki lists one as a Danger Zone reward.
You may want extras, too. Tarkov.help says Final Push's new RDE-78 grenades come from two Therapist barters: one for infected tissue samples and one for Model 8230 CS gas grenades.
What Should You Know Before Entering the Infected Maps?
The official announcement and the wiki confirm the basics, and they shape what you should bring:
- Insurance is off. Anything you leave in the raid is gone. BSG says so directly, and the wiki adds that traders' recovery attempts fail.
- Infected Factory is the short raid. The wiki lists 20 minutes and 4 to 6 players, against 30 minutes and 8 to 10 players on Infected Lab.
- Infected Lab has extra requirements. Entry takes a TerraGroup Labs access keycard, Parking needs Car keys and Sewage Conduit needs the Collector map. The lights are off too. Follow-Up Examination pays 2 keycards.
- Infected Tagilla is out there. He carries a scythe and tarkov.help reports a 100% spawn on the infected maps. Our Tagilla boss guide covers the regular fight.
- Bring cheap kits you can afford to lose. Your survival rate may be low at first. u/lotsofop says they survived 1 of 15 Factory raids.
When Does the Tarkov Halloween Event End?
Battlestate hasn't announced an end date that we could find as of October 30, 2026. The patch 1.3.0.0 notes only list a new Halloween in-game event. How many gas grenades does Therapist give you for First Results? Three RDE-68 grenades when you accept the quest. You can get more through two Therapist barters, which should appear after accepting. If they don't, restart the game.
Why do my kills not count for First Results? The infected has to be under the grenade's effect when it dies. Standing near the cloud or running through it often isn't enough, so let them stay in the gas for a few seconds before you shoot.
Do I need a gas mask for First Results? Yes. Tarkov.help says any gas mask or respirator works, and without one the gas harms you as well.
Can you do First Results in one raid? Yes, according to tarkov.help, though only with good grenade placement. You need an average of 8 to 9 counted kills per grenade.
Can you loot tissue samples from zombies in Tarkov? No, the infected can't be looted. On Therapist's route you get 2 samples from the For the Common Goal step, which comes after First Results, or you buy them on the Flea Market.
