You start at Friendly for free — completing the Black Temple attunement puts you there. From Friendly to Exalted is 39,000 reputation, which is what the counter above is scaled to.
It comes from trash, not bosses. Every Ashtongue kill inside the instance pays; a full clear banks roughly 7,750. That is the whole reason a raid-side service can move this and a solo player cannot — you need repeated instance access with a group that will farm trash on purpose.
What Exalted buys: a class-specific trinket from Okuno, at 28 gold 81 silver — Ashtongue Talisman of Valor for warriors, of Equilibrium for druids, of Swiftness for hunters, of Insight for mages, of Zeal for paladins, of Acumen for priests, of Lethality for rogues, of Vision for shamans, of Shadows for warlocks. Several of them are best-in-slot for the whole phase.
Attunement is required to set foot in Black Temple at all. If your character is not attuned, order the Mount Hyjal and Black Temple attunements first — Hyjal comes before Black Temple, always.
