TL;DR: Is the Diablo 4 Grief Runeword confirmed for Season 16, and what is its recipe? As of November 2, 2026, Blizzard's published Season 16 preview hasn't specifically confirmed Grief's return. The verified Season 15 recipe uses 1 Eth, 1 Tir, 1 Lo, 1 Mal and 1 Ral plus a Common one-handed Sword or Axe in the Horadric Cube. Its unique effect adds 40% bonus Poisoning damage over 5 seconds to direct damage. Those are the current reference numbers while Season 16 remains in testing. Blizzard's Season 16 preview, Grief recipe and effect.

Extra weapon damage, attack speed and poison on one weapon. Grief looks tempting. But spending your runes before checking the base, your build and the season is an expensive mistake. Recognizing the name doesn't answer any of those questions.

Here's what to check: the confirmed recipe, compatible bases, listed stats, full Lo upgrade cost and whether the craft earns its place in your build. Season 15 item data stays clearly labeled, separate from Season 16 announcements.

Is Grief available in Diablo 4 Season 16 yet?

Season 15 is still live; Season 16 is on the 3.3.0 Public Test Realm. Blizzard has scheduled the PTR from October 27 at 10 a.m. PDT through November 3 at 10 a.m. PST. The live patch list has already reached 3.2.4, dated October 28, 2026. Official season page, PTR announcement, live patch notes.

Read the carryover wording carefully. Blizzard says, “some features from the Season of Hell's Legacy will be carrying forward into Season 16.” Those returning features aren't included in this PTR, and more details are promised. Grief isn't named. Blizzard's carryover statement.

So its Season 16 availability is still unresolved.

Absence from this test wouldn't, on its own, prove Grief was removed. A Season 15 recipe page doesn't confirm a Season 16 craft, either. For now, spend around what your current character can actually use. Our Diablo 4 Season 16 PTR guide covers the test schedule and broader changes.

What is the Grief Runeword recipe?

The documented recipe is Eth + Tir + Lo + Mal + Ral + one Common one-handed Sword or Axe. Put one of each rune and the weapon into the Horadric Cube, then transmute them into Grief. Recipe reference.

Before pressing Transmute:

  • Check that the base is Common, with a white item name.
  • Make sure it's a supported one-handed Sword or Axe.
  • Set aside exactly one Eth, Tir, Lo, Mal and Ral.
  • Use an Ancestral Common base if you want the Ancestral result.
  • Confirm your character can equip that weapon type.

Magic, Rare and Legendary bases won't satisfy the documented recipe. Both normal and Ancestral versions consume the same runes; the base quality determines which version you get. D4Guides crafting requirements.

Check the level requirement, too. Item references list a minimum level of 59, and the finished weapon is also subject to its base's level requirement. That matters when you're crafting for an alt. Grief level requirements.

There's a naming wrinkle here: Grief is a crafted Unique weapon, but Diablo IV also uses Runeword for a socketed Ritual Rune and Invocation Rune pairing. That pairing generates and spends Offering. Grief uses the Cube's equipment recipe, so you need the complete five-rune set and white base before crafting. The two rune systems explained.

Do you need to find a Phase Blade first?

A compatible Common Sword is enough. The recipe accepts the weapon category. Grief, Phase Blade is the listed name of the finished sword; Grief, Berserker Axe is the axe variant. D4Guides lists recipes for both Sword and Axe, including their Ancestral versions. Grief recipe variants.

Players raise this exact question in the Phase Blade discussion on r/diablo4. Look for the right white weapon category in your loot. Waiting for a base with the finished Unique's name confuses what goes into the Cube with what comes out.

What are Grief's stats, and what does 40% Poisoning mean?

These are Season 15 reference values from the 900 Item Power Phase Blade listing before upgrades. They aren't announced Season 16 balance values.

The current item references agree on weapon damage, Deadly Strike and attack speed. D4Guides item data, Grief stat reference.

Effect or affixListed valueWhat to check
Bonus Poisoning from direct damage40% over 5 secondsHow much of your damage comes from direct hits
Added Weapon Damage+750–1,000The actual roll on your finished item
Deadly Strike Chance+20%Its value alongside your existing setup
Attack Speed+30–40%Whether your chosen skill benefits from the increase
Damage to Demons+100%The enemy type and your other damage bonuses
Primary Resource on Kill+2Whether the encounter supplies enemies to kill

Bubaigei's Grief database also reproduces the damage-to-Demons and resource values. These numbers describe affixes, not measured total build damage.

Is the poison an instant 40% damage increase?

It takes five seconds. For a simplified example, a direct hit worth 100,000 damage gives you 40,000 at 40%, spread across that duration before other interactions are considered. That's tooltip arithmetic, not a forecast of your final combat damage.

The trigger is direct damage. Existing damage-over-time ticks don't meet that condition, according to the Grief mechanics reference.

Watch how long targets survive. Poison has little room to help when the direct hit already kills a trash monster. Something that lives longer gives it time to contribute. Try both situations before pulling out a weapon that already supports your build.

How do you get the runes for Grief?

Reserve the ingredients you own first. Burn your last Mal upgrading toward Lo and you'll still be missing the Mal that Grief consumes. Write down what's missing before you start converting.

Where can you farm the ingredients now?

For Season 15, the Countess is a documented rune-farming option. You'll find her on the fifth floor of the Forgotten Tower, a Waking Nightmare in Rotspill Delta, Hawezar. Follow the passages down, finish the encounter and collect the rune drops. Countess location and rune-farming guide.

Don't count on Lo. The cited guide gives neither a Lo drop percentage nor an expected run count. Choose a difficulty you can clear reliably, and count the travel through all five floors when deciding whether the farm is paying off.

This route applies to the current season. Its Season 16 usefulness depends on which seasonal encounters and rewards Blizzard brings forward.

Can you craft Lo instead of waiting for a drop?

Yes. The documented Cube recipe is 2 Ohm + 1 Flawless Diamond = 1 Lo. Check the gem quality: Flawless is required, and a Grand Diamond isn't the listed ingredient. Lo crafting recipe.

Starting lower in the chain? Here's the cost at each step:

Cube inputGem inputOutput
2 Mal1 Flawless Amethyst1 Ist
2 Ist1 Flawless Ruby1 Vex
2 Vex1 Flawless Emerald1 Ohm
2 Ohm1 Flawless Diamond1 Lo

Source: Season 15 rune conversion table.

Working through those recipes, one Lo entirely from Mal costs 16 Mal, 8 Flawless Amethysts, 4 Flawless Rubies, 2 Flawless Emeralds and 1 Flawless Diamond. Then Grief takes another Mal. Building Lo from scratch this way brings the bill to 17 Mal total, plus Eth, Tir, Ral and the weapon base.

Count your Ist, Vex and Ohm first. Any you already own reduce that bill.

Don't assume you can upgrade all the way from the cheapest runes, though. The same conversion reference lists no Sol-to-Mal recipe, so it doesn't establish an uninterrupted path to Lo.

For example, 4 Vex need 2 Flawless Emeralds and 1 Flawless Diamond to become Lo. With 2 Ohm, you only need the Diamond. Both totals come from the table above; use the starting point that matches your stash.

Is Grief worth crafting for your build?

Check the weapon slot and your main damage source before chasing a tier-list label. Grief rewards direct damage. It can also displace something your build needs, so compare the whole setup.

Start with three questions:

  • Can your character equip this Sword or Axe variant?
  • What would you lose from the current weapon?
  • Does the poison have time to matter in the content you actually run?

Then take both setups through the same activity. Leave difficulty, skills and other equipment unchanged, and record completion time, deaths and awkward resource gaps. Give each several runs. One lucky burst isn't much of a reason to spend another Lo.

The +100% Damage to Demons line doesn't justify assuming double final damage. Nor should you assume the attack-speed roll speeds up every skill equally. Those are item affixes; your comparison needs to show what they actually do for the build.

Did Blizzard fix Grief's Old Mountain interaction?

Yes. Blizzard's September 22, 2026 Hotfix 4 states: “Fixed an issue where the 5-set bonus for the Old Mountain Talisman combined with Grief resulted in unintended damage.” Official Hotfix 4.

Check any showcase's date. Footage using that interaction before the fix is a poor benchmark for crafting today. The note addresses a specific combination and doesn't publish a new Grief poison percentage.

Which Season 16 changes could affect the decision?

The confirmed preview introduces Horadric Items: Legendary equipment with affixes tied to Talisman sets. Weapon and armor effects activate with at least the relevant two-piece set bonus, while jewelry can add 1 to a specified set's bonus count. Blizzard's Horadric Items explanation.

If Grief returns, that's another contender for its weapon slot.

A Horadric Legendary supporting your chosen set may prove more useful than Grief's listed stats suggest. Keep that in mind when planning, but don't mistake it for a tested Season 16 ranking. Our Horadric Items guide covers those set interactions.

Do the PTR rune changes alter Grief's recipe?

The published adjustments raise Que's Offering requirement from 300 to 500 and Lac's from 400 to 600. Neither belongs to Grief's five-rune recipe. If you use them elsewhere in the build, check their activation requirements separately; these entries don't announce a change to the weapon craft. Official 3.3.0 PTR rune notes.

For an existing Season 15 character, judge the craft by whether it improves the content you're playing now. Planning a fresh Season 16 build? Keep the recipe handy, but wait for explicit availability details before making Grief essential.

FAQ

Is Grief confirmed for Diablo 4 Season 16? Not specifically as of November 2, 2026. Blizzard says some Hell's Legacy features will return, but Grief isn't named in the published PTR preview. This guide's recipe and item values are the verified Season 15 reference. Official PTR preview.

What runes do I need to craft Grief? One Eth, Tir, Lo, Mal and Ral, plus a Common one-handed Sword or Axe. Combine them in the Horadric Cube. Use an Ancestral Common base for the Ancestral version; the rune recipe stays the same. Grief crafting requirements.

Where do I find a Phase Blade for Grief? A compatible Common Sword works as the input. Phase Blade names the finished sword, so don't wait for that exact base name to drop. The alternative recipe takes a Common Axe. Sword and Axe recipes.

How do I get Mal runes for Grief? The Countess in the Forgotten Tower is a documented Season 15 rune-farming route. The published conversion table has no Sol-to-Mal step. Reserve one Mal for Grief before upgrading duplicates toward Lo. Rune sources and conversions.

Can I craft Lo with two Ohm runes? Yes, but you also need one Flawless Diamond in the Horadric Cube. Two Ohm alone won't complete it. Making both Ohm from Mal through the full chain costs 16 Mal plus the required gems for Lo. Lo recipe.

Does Grief's poison trigger from damage-over-time ticks? Direct damage is the condition. Existing poison, bleed or burn ticks don't qualify simply because they deal damage. Assess your build's direct-hit portion instead of applying the 40% figure to every source. Grief effect explanation.
