What Headhunter actually does

Headhunter is a unique Leather Belt. Its full modifier list is:

  • +(40–55) to Strength
  • +(40–55) to Dexterity
  • +(50–60) to maximum Life
  • (20–30)% increased Damage with Hits against Rare monsters
  • "When you Kill a Rare monster, you gain its Modifiers for 60 seconds"

That last line is the whole reason the belt is chase-tier. Every time you kill a rare (yellow) monster, you steal its mods (extra speed, extra damage, elemental bonuses, aura effects) for a full minute. In content packed with rare monsters, those buffs stack on top of each other, and you snowball into a blur of movement and damage that clears screens in a single hit.

One important correction if you've read older guides: the buff duration is 60 seconds, not 20. It was tripled from 20 to 60 in patch 3.18.0e, whose official note is literally titled "Headhunter now steals mods for 60 seconds and a wider variety of rare modifiers will spawn in areas." Any guide still saying 20 seconds is at least seven leagues out of date.

The buff is worthless against white and magic monsters and against bosses (they aren't rares), so Headhunter is a mapping and clear-speed item, not a boss-killing one. That's also why it pairs with rare-dense mechanics — more on that below.

How to get Headhunter in PoE 3.29

There are two honest routes: hope it drops, or farm The Doctor. Everything else is a variation on those.

Route 1: the raw item drop (luck)

Since Content Update 3.14 (Ultimatum), Headhunter was moved into the core drop pool. It can now drop from any monster in an area of level 40 or higher, at a very low base rate. You do not need any special league modifier for it to drop.

This matters because a lot of old advice tells you to "run Nemesis-modded maps to get Headhunter." That was true years ago: before 3.14, Headhunter only existed inside the Nemesis league modifier. That gate is gone. Don't waste currency chasing a Nemesis mod for this belt. A raw Headhunter can drop off any rare or magic-pack monster in the endgame; it's just rare enough that you can't rely on it.

Because the base drop rate is so low, most players never see one drop naturally. That's why the community farms the card instead.

Route 2: The Doctor divination card (the real farm)

The reliable, buildable route is the The Doctor divination card:

  • Set size: 8 cards. Collect 8 copies of The Doctor and turn them in for a guaranteed Headhunter.
  • Drop location: Burial Chambers Map. The Doctor drops there, so you spec your Atlas toward Burial Chambers and farm it repeatedly.
  • Turn the completed set in at any divination card trade — the card-exchange NPC in your hideout, or Navali — to receive the belt.

This is a genuine grind: The Doctor is a rare, high-value card, so a full 8-set represents a lot of maps. But it's deterministic — you know exactly what you're working toward, unlike praying for a raw drop. To make Burial Chambers farming pay, favourite the map on your Atlas, spec into divination-card and pack-density nodes, and run divination-focused Scarabs to multiply card drops. A handful of other cards can also award Headhunter incidentally (The Fiend, The Valkyrie, The Undaunted, The Demon), but they aren't efficient farm targets — The Doctor is the one everyone farms, and the only one worth building an Atlas around. Did 3.29 change anything about Headhunter?

No. This is the reassuring part: nothing in the 3.29 "Curse of the Allflame" cycle touched Headhunter, The Doctor card, the Burial Chambers Map, or the core-drop mechanic. The official 3.29.0 patch notes contain no mention of the belt or its card, and the follow-up patches (3.29.1 through 3.29.3b, the latest live patch shipped 4 September 2026) were bugfix and quality-of-life drops with no changes to it.

For context, 3.29 did shuffle some divination cards — a few were temporarily disabled and had turn-ins paused — but The Doctor was not among them. The Doctor farm has been stable all league, and will keep working until the next league resets the Atlas.

That stability is exactly why Headhunter is worth learning once and farming whenever you want it: the acquisition method doesn't rot the way league-specific currency strategies do.

What is Headhunter worth, and should you farm or buy it?

Headhunter is a multi-divine chase item — well into the "you'll feel it" price bracket, but nowhere near mirror-tier the way a fully-rolled Mageblood or a Mirror of Kalandra is. Its price swings hard over a league: expensive in the first weeks when everyone wants one for their magic-find mapper, cheaper deep into the league as supply catches up.

What builds use Headhunter?

Headhunter is a clear-speed and magic-find belt above all else. As PoE Vault puts it, it "provides a significant boost to all aspects of a player's abilities and defences when used in high budget builds purely focused on obliterating entire maps."

The classic home for it is a Magic Find (MF) mapper. Those builds, in PoE Vault's words, are "most commonly known to be used with Magic Find builds due to the fact that they sacrifice a lot of defences for Increased Item Quantity and Rarity to steal rare mob's defensive modifiers." In other words, the belt lets a glass-cannon farmer borrow toughness from the monsters it kills — the stolen rare mods plug the defensive holes the build left open for more loot.

Beyond dedicated MF, any fast-clearing mapper benefits: the more rare monsters on screen, the more buffs stacking at once. That's why Headhunter is happiest in rare-dense content — Legion encounters, Breach waves, Delirium fog and Harbinger packs all spawn walls of rares that keep the buff pool topped up. Common mistakes to avoid

  • Don't chase the Nemesis mod. The Nemesis-league gate on Headhunter was removed in 3.14. It drops from core loot in any level-40+ area now — running Nemesis-modded maps is not a Headhunter strategy.
  • Don't believe "20 seconds." The buff is 60 seconds since 3.18.0e. Guides quoting 20s are badly out of date.
  • Don't confuse it with Path of Exile 2's belt. PoE2 has a Headhunter on a different base type (a Heavy Belt) with different numbers. This guide is for the original Path of Exile, where the belt is a Leather Belt.
  • Don't farm the wrong map for The Doctor. The Doctor drops in Burial Chambers, not the Sepulchre / Putrid Cloister / Waste Pool areas some older lists cite (those apply to different cards).
